Join Our Team as a Lead Pharmacist for Medicines Optimisation Services
The Medicines Management Team at CLCH is seeking a dynamic and innovative Lead Pharmacist to lead Medicines Optimisation Services. This role involves shaping clinical services within the Trust, including leadership of pharmacists working in primary care networks. The post holder will support the Chief Pharmacist in implementing recommendations from the NHS 10-year plan and developing the service further.
The successful candidate will manage the team, including staff management, prioritising work, and identifying future organisational needs. The post also requires deputising for the Deputy Chief Pharmacist and Chief Pharmacist as needed.
Main Duties
1. Lead the development and integration of clinical medicines optimisation services across CLCH, including care homes, primary care networks, and domiciliary settings.
2. Line manage multiple direct reports.
3. Provide professional advice, support, and training on policies and practices to health professionals in community care and related agencies.
4. Contribute to clinical governance and patient safety initiatives through interventions that optimise medicine management.
5. Develop and maintain a comprehensive medicines management service for vulnerable client groups in the community.
6. Deputise for the Deputy Chief Pharmacist and/or Chief Pharmacist as required.
