Social Worker | Full Time | Permanent | Intermediate Care Team | Thornton, Lancashire
Lancashire County Council is recruiting a permanent Social Worker to join the Intermediate Care Team in Thornton, Lancashire. This role offers 37 hours per week, salary £36,363 - £40,778 per annum, on a 7-day contract including weekend working (1 in every 4 weekends).
Responsibilities
* Provide holistic, face-to-face Care Act assessments, reviews and support planning using a strengths-based approach.
* Work across Lancashire as part of the Urgent Care Acute and Prisons Service, supporting hospital discharges and facilitating care to help people live independently at home.
* Collaborate with the multi-disciplinary team and use the Discharge to Assess model to ensure timely assessments away from hospital.
* Support people to achieve optimum independence and their chosen living arrangements.
* Contribute to a 7-day service within the Intermediate Care Team including reablement and residential rehab.
Qualifications and Requirements
* Be registered with Social Work England (SWE) or have applied for registration.
* Essential social work qualifications and driving requirement if required (car for duties; insured for business use; consideration for disability).
