Bridge End Surgery is seeking an experienced Advanced Nurse Practitioner to join our clinical team.
The ideal candidate will be highly motivated, possess excellent communication and interpersonal skills, and have completed an Independent Prescriber qualification. Primary care experience is advantageous.
Main duties of the job
Provide clinical practice including assessment, diagnosis, planning, implementation, and evaluation of treatment and care for patients with undifferentiated diagnoses. Examine and assess patient needs from physiological and psychological perspectives, and plan care accordingly.
Identify, diagnose, and manage treatment plans for patients at risk of long-term conditions. Manage both acute and chronic diseases, integrating drug and non-drug treatment methods.
Provide medication advice, including prescribed and over-the-counter drugs, regarding regimes, side effects, and interactions.
Prioritize health problems and intervene in complex, urgent, or emergency situations, including initiating emergency care.

We are a well-established practice in Chester le Street, part of the Chester le Street Primary Care Network, serving approximately 8,000 patients.
The clinical team includes 2 GP Partners, 6 salaried GPs, 1 Advanced Nurse Practitioner, 2 Practice Nurses, 1 Healthcare Assistant, 1 Nurse Associate, and 1 GPA. Our reception and admin team are highly experienced.
Bridge End Surgery is an established training practice.
Job responsibilities
The post holder will provide patient care from initial history taking through diagnosis, treatment, and evaluation, working within their professional boundaries and demonstrating safe, clinical decision-making and expert care.
Qualifications for advanced practice as per NMC standards are required.
