Bridge End Surgery is seeking an experienced Advanced Nurse Practitioner to join our clinical team.

The ideal candidate will be highly motivated, possess excellent communication and interpersonal skills, and have completed an Independent Prescriber qualification. Primary care experience is advantageous.


Main duties of the job

Provide clinical practice including assessment, diagnosis, planning, implementation, and evaluation of treatment and care for patients with undifferentiated diagnoses. Examine and assess patient needs from physiological and psychological perspectives, and plan care accordingly.

Identify, diagnose, and manage treatment plans for patients at risk of long-term conditions. Manage both acute and chronic diseases, integrating drug and non-drug treatment methods.

Provide medication advice, including prescribed and over-the-counter drugs, regarding regimes, side effects, and interactions.

Prioritize health problems and intervene in complex, urgent, or emergency situations, including initiating emergency care.


About us

We are a well-established practice in Chester le Street, part of the Chester le Street Primary Care Network, serving approximately 8,000 patients.

The clinical team includes 2 GP Partners, 6 salaried GPs, 1 Advanced Nurse Practitioner, 2 Practice Nurses, 1 Healthcare Assistant, 1 Nurse Associate, and 1 GPA. Our reception and admin team are highly experienced.

Bridge End Surgery is an established training practice.


Job responsibilities

The post holder will provide patient care from initial history taking through diagnosis, treatment, and evaluation, working within their professional boundaries and demonstrating safe, clinical decision-making and expert care.

Qualifications for advanced practice as per NMC standards are required.

Key Responsibilities

Clinical Practice

* Assess, diagnose, plan, implement, and evaluate treatment for undifferentiated diagnoses.
* Examine and assess patient needs from physiological and psychological perspectives.
* Manage acute and chronic conditions, integrating drug and non-drug treatments.
* Prescribe and review medications following evidence-based practice and protocols.
* Prioritize health problems and intervene appropriately, including emergency care.

Communication

Use sensitive communication styles, ensure informed consent, and adapt communication methods for diverse patient backgrounds.

Maintain effective communication within the practice and with external stakeholders, acting as an advocate for patients and colleagues.

Delivering a quality service

Work within your competence and NMC code of conduct, maintain accurate records, and deliver evidence-based care.

Prioritize and organize workload, evaluate care effectiveness, and collaborate on quality improvements.

Participate in shared learning, manage patient complaints and incidents, and engage in continuous professional development.

Team Working

Understand your role, support team members, develop new ways of working, and create effective referral mechanisms.

Manage time effectively and work collaboratively to improve patient care.

Management of risk

Assess and manage risks, ensure infection control, and advocate for policies reducing health risks.

Managing Information

Use technology and software for planning, monitoring, and communication; ensure accurate data coding for audits.

Learning and development

Share learning with the team and pursue personal development needs.

* Registered first-level nurse or equivalent
