Overview
We are looking for a pharmacist with a proven track record supporting acutely unwell medical and surgical patients to work in the Virtual Hospital in West Essex.
The Virtual Hospital team cares for acutely unwell patients in homes, enabling them to remain at home by preventing unnecessary admissions and supporting early discharge. It utilises remote technology to provide a safe alternative to hospital admission.
Responsibilities
* Provide specialist clinical pharmacy support to the Virtual Hospital, delivering medicines‑related care of patients.
* Support the provision and development of medicines‑management services.
* Participate in training, education, and research as required.
* Contribute to the organisation and management of the clinical pharmacy service.
* Provide clinical pharmacy and medicines‑management advice to maximise benefits and minimise risks.
* Participate in clinical audit.
* Supervise junior colleagues where required.
* Perform responsible pharmacist duties and out‑of‑hours services, including weekend and bank holiday dispensary rota, on‑call rota, and the pharmacists rota.
Qualifications
* Registered with the GPhC.
* Demonstrate clinical credentials.
* Pass required pre‑employment checks including COVID‑19 vaccination (both doses), unless exempt.
