Our Stoke Community Drug and Alcohol service is looking for a Hospital Link Worker to join our team. If you are enthusiastic, creative, and passionate about helping others, we'd love to hear from you.
Main duties of the job
The Hospital Link Worker will provide support for people in hospital who are using drugs or alcohol and want to engage in treatment. You will form links with the hospital to ensure continuity of care, establishing and maintaining a quality hospital liaison service, and developing inpatient and outpatient services for those with substance-related issues. You will liaise between:
* Hospital and community pharmacies
* Hospital safeguarding teams, inpatient and community Mental Health Teams
* Accident & Emergency to provide early interventions for those with drug or alcohol-related health concerns
* core WithYou teams including our Clinical Lead
This is an integral role for With You. As a frontline worker, you will deliver continuity of care through effective partnership working, care coordination, delivery of interventions, and key work responsibilities. Job responsibilities

This is a full-time, fixed-term position working 37.5 hours per week until 31/03/2026. Some Saturday working is required on a rota basis, and the salary is £23,821 to £26,500 per annum.

Experience
* We're looking for a compassionate and motivated individual with an exceptional ability to build meaningful relationships.
* You will have demonstrable experience working within the health, social care, criminal justice, or other specialist settings, or a clear motivation for wanting to work in the drug and alcohol sector.
* A successful track record of providing efficient and reliable customer service is essential, and you will also be organized with excellent time management skills.
