Job Purpose
Mind in the Vale of Glamorgan is seeking an experienced, grounded Mental Health Practitioner to deliver high-quality, recovery-focused mental health support to people aged 16+ within community settings across the Vale of Glamorgan.

This role is suited to someone with at least two years' experience working in mental health who is confident working independently in the community, applies sound professional judgement, and understands how to balance compassion with practical service delivery. The post holder will deliver guided self-help interventions, social prescribing, structured group courses and one-to-one support, working collaboratively across community, primary and secondary care pathways and alongside voluntary and statutory partners.
The role requires a practitioner who is comfortable working flexibly across community venues, values clear communication and teamwork (including with administrative and coordination staff), and is committed to evidencing impact through meaningful holistic and psychological outcome measures.
Key Responsibilities
* Deliver guided self-help interventions using Centre for Clinical Interventions (CCI) materials
* Facilitate and deliver structured mental health and wellbeing courses
* Provide one-to-one mental health support within community-based settings, including outreach venues and partner locations
* Work across community, primary and secondary care pathways
* Deliver social prescribing interventions and support people to access appropriate community resources
* Represent Mind in the Vale of Glamorgan at multi-agency meetings, forums and community events
* Work flexibly across premises and community settings throughout the Vale of Glamorgan
* Use holistic and psychological outcome measures to monitor progress and demonstrate impact
* Maintain accurate, timely and confidential records using case management systems
* Work in line with organisational policies, safeguarding requirements and governance processes
* Communicate effectively and work collaboratively with administrators, co-ordinators and colleagues to ensure smooth service delivery
* Apply professional judgement and practical problem-solving while working independently within the community
Person Specification
Essential Criteria
* At least two years' experience working in a mental health setting
* Experience supporting people with a range of mental health needs
* Experience of delivering mental health support within community-based services
* Understanding of recovery-focused, person-centred approaches
* Knowledge of guided self-help interventions
* Understanding of social prescribing and community-based support
* Ability to deliver both group courses and one-to-one mental health support
* Willingness and ability to work independently within community settings, using sound professional judgement
* Strong communication skills and ability to work effectively within multi-agency partnerships
* Ability to manage a caseload effectively within a 37-hour working week
* Competent use of IT systems, databases and Microsoft Excel
* Willingness and ability to work across multiple community sites
Desirable Criteria
* Experience working in the voluntary or third sector
* Experience delivering CCI-based interventions
* Experience working across primary and/or secondary care pathways
* Experience of using outcome measures, monitoring impact and contributing to service evaluation and improvement
