The Role
Within Locala’s Health and Wellbeing Integrated Communities, we deliver multidisciplinary community services, including integrated health and social care in collaboration with Kirklees Council.
We seek a motivated and enthusiastic Staff Nurse to join our Intermediate Care Team. The service supports patients discharged from hospital, assisting their journey home. Responsibilities include wound care, monitoring deteriorating patients, and more. This role offers opportunities for skill and professional development in a supportive environment.
The Nursing Team collaborates with GPs, Physician Associates, Occupational Therapists, Physiotherapists, Pharmacists, Healthcare Support Workers, and the Kirklees Care Team. Our goal is to rehabilitate service users in their homes, prevent hospital admissions, and ensure safe discharges.
We operate a 7-day service from 8 am to 7 pm on a rota basis, providing holistic, patient-centered assessment, treatment, and evaluation to prevent unnecessary hospital admissions and facilitate swift discharges, enabling recovery at home and avoiding premature long-term care admissions.
