An exciting opportunity has arisen to join an innovative team within the Serious Mental Illness (SMI) and frailty pathway in NE Essex. We are looking to recruit a motivated, hardworking team leader who has excellent communication skills with a passion in driving service improvement and working collaboratively with other health care providers and third sector agencies. The successful candidate will be responsible for the delivery of a high level service in providing a proactive assessment, monitoring and management of patients over the age of 70 years with an SMI and the management of the team. As part of the national agenda for transformation the service will focus upon admission avoidance to both general hospital and a mental health inpatient setting by integrated working with the community teams in line with the North East Essex Alliance.
1. Responsible for the overall delivery of a range of evidence based interventions to service users based on identified needs, review the effectiveness of care packages and discharge service users back to primary care / other providers in a timely manner.
2. To ensure the service works in line with agreed pathways, National and Trust policies and guidelines, and is focused on delivery of outcomes. This is a developmental role where the team lead and the team will be required to be adaptable to changes within service development.
3. Implement a team approach to complex or sensitive case management where practitioners are not isolated in decision making and have the ability to access a range of expertise to inform decision making.
4. Develop robust interagency relationships with key stakeholders supporting the development of the integrated care pathways and locality working.
5. Assist with the efficient and effective management of budgets. With responsibility as an authorised signatory to handle cash & valuables, small payments and to maintain stock control by ordering supplies when necessary.
6. Responsible for the coordination and management of the team in providing a safe, effective and therapeutic clinical service, ensuring the delivery of high quality community mental health care packages.
