Advanced Nurse Practitioner - Palliative Care

An exciting opportunity has arisen for anexperienced Advanced Nurse Practitioner (ANP) to join our palliativecare team at Woking & Sam Beare Hospice and Wellbeing Care, basedin our bespoke facility in Goldsworth Park, Woking. This is a pivotal timefor the Hospice as we develop new models of care, and we are seeking an ANPwith extensive clinical experience and proven leadership skills to help shapethe future of palliative and end of life care across Northwest Surrey.

As an integral member of our multidisciplinaryteam, you will work within the medical team, report tothe Consultants in Palliative Care, and collaborate closely withour nursing teams. You will deliver expert, holistic care across our InpatientUnit, Community Services, Outpatient Clinics, and the WellbeingCentre, ensuring seamless care coordination and high standards of symptommanagement. Your clinical expertise will be matched by your ability tocommunicate effectively, work autonomously, and lead with compassion andinnovation.


Main duties of the job

Key Responsibilities

* Lead on complex symptom management,prescribing, and advance care planning
* Undertake autonomous assessments,diagnoses, and treatment planning
* Provide urgent clinical support inthe community, including rapid response visits and crisis intervention
* Mentor junior staff and contribute tostrategic service development, education, and governance
* Participate in a 1 in 4 or 1 in 5 on-callrota, supporting both inpatient and community services
* Support service innovation andcontribute to audit, research, and policy development
* Work collaboratively with NHSpartners and local healthcare providers to ensure continuity and integration ofcare.

* Registered Nurse on the NMC Register
* Advanced Clinical Assessment
* Minimum of four years post registration

* Advanced Communication or equivalent
* experience
* Management or Leadership
* certificate
* Teaching qualification
