Job Overview
The Cardiac Rehabilitation Service at Leighton Hospital is seeking a Band 7 Clinical Nurse Specialist or Allied Health Professional to lead the development, implementation, and evaluation of a seamless specialist service, ensuring the highest standards of clinical care. The post holder will demonstrate expertise within the specialist service, providing advice, education, and support to staff, patients, families, and carers. Active participation in clinical activities and audits within their specialist area is required.
Key Responsibilities
1. Liaise with clinical colleagues across MCHFT, Primary Care, and external centres to promote a cohesive cardiac rehabilitation pathway from discharge to long-term health maintenance.
2. Lead a specialist practice, providing expert advice within guidelines and protocols, including collaboration with medical colleagues and other professionals.
3. Ensure deployment of clinical and managerial skills of the team to provide comprehensive rehabilitation for patients post-acute coronary syndrome, revascularisation, or valve surgery.
4. Deliver patient-centered, evidence-based programmes reflecting BACPR standards.
5. Coordinate and develop the rehabilitation team to deliver education and exercise programmes at Leighton Hospital and off-site locations.
6. Facilitate data collection for the National Audit of Cardiac Rehabilitation, supporting data submission and reporting.
7. Utilize electronic systems to ensure excellent communication across secondary and primary care interfaces.
8. Promote evidence-based practice and foster team development in response to service innovations.
9. Create a learning environment for staff development and supervise students from various disciplines.
