As a Practice Nurse, we would be looking to provide afull range of nursing services. Competencies including Chronic Disease Management, INR dosing, childhoodimmunisations, smear taking, travel vaccinations etc.

Main duties of the job
Clinical practice
Assess, plan, develop, implement and evaluate treatmentprogrammes that promote health and well-being.
Assess, plan, implement and evaluate individual treatmentplans for patients with a known chronic condition.
Proactively identify and manage treatment plans for patientsat risk of developing a chronic condition as appropriate.
Work autonomously and with other health care professionalsto diagnose, monitor, manage and treat chronic conditions, includingnon-drug-based treatment methods using a management plan, and in line withnational and local policies and healthcare service delivery.
Support patients to adopt health promotion strategies thatpromote principles of self-care.
Implement and participate in vaccination and immunisationprogrammes (adults and children).
Assess, plan, develop, implement and evaluate programmes topromote health and well-being, and prevent adverse effects on individualshealth and well-being.
Implement and evaluate individual treatment plans forpatients with a known chronic condition.
Support patients to adopt health promotion strategies thatpromote patients to live healthily and encourage principles of self-care.
Assess and care for patients presenting with bothcomplicated and uncomplicated wounds.
Advise, support, screen, implement and participate invaccination and immunisation programmes for patients.

Job responsibilities
The post holder is responsible for the delivery of effectiveprimary health care and chronic disease management to the patients of Strathmore Medical Practice. The focus of the role is both the delivery ofevidence-based practice for patients presenting with complex and chronicconditions, and the provision of preventative health care to the practicepopulation.
As an autonomous practitioner the post holder is responsiblefor the care delivered, demonstrating critical thinking and skills in clinicaldecision-making in the management of patients.
Reporting to the Lead Senior Nurse, you will work collaborativelyacross the primary healthcare team to meet the needs of patients, support thedelivery of healthcare and public health within primary care working withindefined policy and procedures.
Person Specification
Knowledge and Skills
* Post qualification registration experience/ within primary care Knowledge of health promotion strategies and ability to apply to clinical practice. Knowledge of patient group directions and associated policy. Knowledge of needs of patients with long term conditions. Experience of quality initiatives such as clinical benchmarking and audit.
* Experience of working within primary care Experience of working within a team Experience of working autonomously
Qualifications
* Diploma/Degree level study or equivalent relevant to post. Registered Nurse with NMC Evidence of continued professional development
* Experience within General practice setting/primary care
