We are looking for an enthusiastic and motivated Mental Health Link Worker to work in the community mental health service in Wigan Living Well (LW). The Living Well Service will enable timely access to mental health services when people need it most and provide support for people who might not traditionally meet the threshold or criteria of existing mental health services.
The team will be multidisciplinary and will include third sector organizations.
We will also be supporting several peer mentor roles within the team.
The team work and cover the whole of the Wigan Borough.
To contribute to the service delivery and target objectives/priorities of the LW Service, to ensure the most efficient contribution of knowledge and practice.
To work with colleagues in the completion and implementation of risk assessments, formulations, and risk management plans for people in the LW service.
To work with colleagues in the LW service to implement agreed shared plans and interventions, particularly OT. To contribute to the clinical discussion of people using the LW service through attendance at regular MDT meetings.
To identify and respond appropriately to child protection issues, as described with the guidelines of Local Safeguarding Children’s Board and Greater Manchester Mental Health Foundation Trust safeguarding policies.
To maintain clear and comprehensive electronic clinical records, to ensure information is accurate and complies with Data Protection Act and Trust standards.
To provide written reports detailing the progress of mental health interventions.
To engage with using routine outcome measures as clinically appropriate including their use in clinical practice and inputting data into clinical records.
To identify and carry out appropriate liaison and outreach work in partnership with other agencies to provide socially inclusive opportunities for people who use the LW service.
To provide information for performance management purposes as requested by the Manager Greater Manchester Mental Health (GMMH) Foundation Trust employs over 7000 members of staff, who deliver services from more than 122 locations.
We provide inpatient and community‑based mental health care for people living in Bolton, the city of Manchester, Salford, Trafford and the borough of Wigan, and a wide range of specialist mental health and substance misuse services across Greater Manchester, the north west of England and beyond.
