Shift Pattern: Mon - Fri – Start from 8am onwards, finish 5.30pm at the latest
Pay Rates – Day: £22.99 per hour

Responsibilities of this job role:
· Provide a dietetic service and give nutritional advice to adult and paediatric patients, with a range of conditions, in the primary care setting. This includes the assessment and treatment of patients and working as an integral member of the MDT to provide a flexible, prioritised service based on clinical need.
· Be an integral member of the team sharing the caseload for home enterally fed patients both in the clinic setting and conducting home visits for patients in their own homes/nursing homes.
· Set up and lead specific patient caseload groups to facilitate and educate in key aspects of clinical nutrition as required.
· Design individual client-centred treatment plans. This will involve nutritional assessment and calculation of nutritional requirements based on the interpretation of biochemistry, anthropometry, clinical condition and diet history.
· Negotiate on-going dietary adjustments with patients based upon the patient's agenda, biochemistry, metabolic status, clinical condition and priorities.
To be successful in applying for this role you will need:
· HCPC registered.
· BSc (Hons) Nutrition / Dietetics degree or post graduate diploma or equivalent.
· Previous NHS experience is desirable but not essential.
· Ideally looking for someone on a F-T basis or minimum 3 days per week, remote working is also an option.
