In this role, you'll deliver high-quality, evidence-based respiratory nursing care to patients with chronic respiratory conditions, helping to improve quality of life, promote independence, prevent avoidable hospital admissions, and support timely discharge from acute services.You'll work closely with senior clinicians, the wider multidisciplinary team, and primary care colleagues, contributing to proactive case management and health promotion initiatives aligned with neighbourhood and community care models.Your base will be Livingstone Hospital, Dartford, with regular travel across Dartford, Gravesend and Swanley.
