About The Role
Rated in our inspection by CQC as ‘GOOD’, we currently have vacancies for new and experienced qualified social workers. Hounslow is a wonderfully diverse Borough. As a qualified social worker within Hounslow, you will have exciting opportunities to work within diverse teams across adults social care.
Working within the full legal framework of social work functions, you will assist residents and carers to identify needs, assets, strengths and desired outcomes, helping them to formulate a care plan/support plan. Our emphasis is on person‑centred, assets‑based, relationship‑based practice.
Key Responsibilities
* Assist residents and carers to identify needs, assets, strengths and outcomes.
* Formulate care plans or support plans, using personal budgets where appropriate.
* Protect the human rights of residents and carers and promote safety in line with safeguarding policy.
* Formulate protection plans, carry out safeguarding enquiries and participate in outcome meetings.
* Collaborate with multi‑agency teams, including review teams, hospital based team, AMHP service, and learning disability services.
Our Department & Teams
* First Contact Team – triage and signpost unallocated queries.
* Locality Teams – long‑term support, safeguarding enquiries, capacity assessments.
* Review Team – safeguard issues, capacity assessments, court of protection work.
* Hospital Social Work Team – support for residents leaving hospital with immediate service plans.
* AMHP Service – day services for adults under the Mental Health Act, CTOs and breathing space referrals.
* Adult Learning Disability, Autism Service (ALDAS) and Independent Futures – support adults and younger residents with learning/Autism needs.
Supporting Your Development
Regular reflective one‑to‑one supervision. Opportunities for specialist training (Best Interest Assessor, Practice Educator, AMHP, leadership training). Corporate benefits and free Social Work England registration fee payment.
About You
* Passionate about promoting human rights and person‑centred practice.
* Experience working with vulnerable adults and strong advocacy skills.
* Robust knowledge of the legal framework for adult social care.
* Competent written and verbal communication skills.
* Partnership working skills with agencies, voluntary sector, and others.
* Understanding of safeguarding and health & social care legislation.
Qualifications
* Minimum: Masters/BA in Social Work, DipSW, CQSW, CSS or equivalent.
* Social Work England Registration.
* Enhanced DBS.
Desirable
* Specialist qualifications e.g. Best Interest Assessor, Approved Mental Health Professional, Safeguarding.
