The Role
As Nurse Case Manager, youll take ownership of complex care packages for adult clients, ensuring safe, high-quality delivery while developing and supporting care teams.
Key responsibilities:
Managing and reviewing complex care packages in the community
Leading clinical quality, governance, and best practice
Delivering specialist training (e.g. tracheostomy, ventilation)
Assessing and signing off staff competencies
Coaching and mentoring care teams
Building strong relationships with clients, families, and MDTs
About You Registered Nurse with NMC PIN
Experience in complex care (e.g. tracheostomy, ventilation, spinal injury)
Passion for developing others and improving care standards
Strong communication and organisational skills
Confident working independently in a community setting
Full UK driving licence and access to a vehicle
