An exciting opportunity has arisen for dynamic, motivated and innovative pharmacists to join our expanding Medicines Management team at Central London Community Healthcare Trust.
We are looking for part‑time and full‑time pharmacists to provide a clinical pharmacy service to our inpatients in our Rehabilitation Service across the Trust.
Key Responsibilities
As an integral member of the team, you will be providing specialist expertise in the use of medicines whilst supporting the address of both public health and social care needs of patients. You will be involved in conducting structured medication reviews for patients with polypharmacy, long‑term conditions or high‑risk patients, ensuring an integrated approach to the management of medicines.
If you are successful you will join the existing team of Band 7 pharmacists at the Trust covering a range of rehabilitation specialties. You will be supported by experienced Senior Clinical Pharmacists, allowing you to gain new skills in each rotation and the ideal opportunity to develop strategic, quality‑improvement and leadership skills.
The post holder may be required to travel across the different sites within the Trust where we have Bed‑ed Services inpatients, so will require some flexibility.
