Overview
An exciting opportunity has arisen at Northamptonshire Healthcare NHS Foundation Trust. We are looking for a Health Care Assistant to join our team. The service aims to provide an excellent standard of person-centred planned and unplanned nursing intervention to patients in their homes and is part of an integrated service that supports patients to remain in their home environment. The successful candidate will deliver safe, effective care for the patient experience. You will work closely with the multi-disciplinary team to support the service with a focus on delivering quality outcomes and improving the patient experience. You should be enthusiastic, innovative and highly motivated, with the ability to be flexible to the needs of the service and deliver a high standard of care in a supportive and forward-thinking environment. The service is provided from 8am to 6pm seven days a week, over a shift pattern. Weekends and bank holidays are a requirement of the job. Car owner/driver and a current driving license are essential or access to a car (unless you have a disability as defined by the Equality Act 2010).
Responsibilities

Your patients could be any age, but they’ll often be elderly; others may be recently discharged from hospital, terminally ill or have physical disabilities. The service aims to provide an excellent standard of care that supports patients. In Northamptonshire you will work as part of a multi-disciplinary team in order to support the service and focus on delivering quality outcomes and improving the patient experience.
