Job Overview
We are delighted to offer this secondment opportunity to an experienced Community Sister/Charge Nurse to cover a 6‑month maternity leave at FHFT. You will lead the nursing team, line‑manage Community Sisters, and be clinically responsible for safe delegation of duties to Rehab Support Workers, ensuring early deterioration is identified.
Main Responsibilities
* Provide expert clinical nursing leadership within community services and Integrated Care Teams, ensuring high rehab standards and avoiding unnecessary secondary care admissions.
* Undertake a professional nursing leadership role in Intermediate Care and act as a clinical exemplar.
* Collaborate with primary, secondary, and social care partners to support an enhanced recovery model.
* Act as an autonomous clinician accountable for both own and delegated tasks.
* Negotiate individual roles and responsibilities with patients, carers and other professionals; refer appropriately.
* Contribute to service strategy and clinical standard operating procedures.
* Establish local networks with other health and social agencies and national links to develop protocols in line with national and local guidelines.
Qualifications
* Degree or equivalent in Nursing.
* Registered adult nurse with the NMC.
* Physical assessment module.
Desirable Qualifications
* Teaching and assessing qualification ENB 998 or equivalent.
* Post‑registration qualification.
Experience
* Significant post‑registration experience, including a senior nursing position.
* Experience of caseload management with complex care packages for vulnerable people.
* Experience in long‑term conditions and end‑of‑life care.
* Understanding and insight into current NHS issues and developments.
Skills

* IT skills – computer literate.
* Organisational skills, including decision making and prioritising.
