Post No. SSAC0110.4 - Assistant Team Manager We are looking to recruit an exceptional Assistant Team Manager to help lead and shape our hospital-based discharge service for Newport. This is a fantastic opportunity for a motivated, skilled and compassionate professional to play a vital role in supporting people to leave hospital safely, promptly and with the right support in place. Our Newport Hospital Discharge Service is a dedicated, fast-moving team based at the Royal Gwent Hospital site, working across hospital settings to support Newport residents at key points in their recovery and discharge journey. The team brings together qualified and non-qualified staff who work collaboratively with health colleagues and wider partners to achieve positive, person-centred outcomes for individuals and carers. You will also work closely with Home First, a specialist team hosted by Newport that provides a rapid response service on behalf of the five local authorities in Gwent. This close partnership strengthens our discharge pathways and creates a genuinely integrated, multi-agency approach focused on reducing delays and promoting independence from the earliest possible stage. As Assistant Team Manager, you will play a key leadership role in supporting and developing effective hospital discharge arrangements in a challenging and rewarding environment. This is an exciting opportunity to influence service delivery, strengthen pathways, support practice improvement and help shape innovative responses in a dynamic, fast-paced setting where no two days are the same. The successful candidate will hold a relevant professional qualification, such as Social Work or Occupational Therapy, and will bring strong leadership skills, sound professional judgement and a commitment to high-quality, strengths-based practice. You will provide visible on-site support to staff, work confidently with key stakeholders across health and social care, and support the Team Manager with the operational management, development and continuous improvement of the service.
