Job Overview
Are you an established Occupational Therapist, Mental Health Nurse or Practitioner looking to work in the community? Intensive Community Support Dementia team would like to hear from you! The community role covers North and West Dorset, being a driver is essential for this role, pool cars are available. As one of the Clinical Team Leaders you will be supporting patients in a specialist service in which Community Mental Health Teams can refer to when a patients behaviour becomes more challenging. We offer high quality interventions to help patients in their own environment, this could be either a care/residential home or in their own private home. You will work with CMHTs, social care and other agencies to benefit patients and support carers while patients are under our care and signpost for ongoing support. You will be highly motivated and confident in clinical decision-making. You will undertake shift management ensuring the smooth running of the team, using resources effectively.
Working Hours
This role is a part‑time post of 30 hours per week, we would welcome part time applicants with 30 hours. ICSD is a 7 day a week service, 07:30‑20:00. We work on a rota basis 07:30 -15:30/12:00 20:00 covering weekends at enhanced pay ! There is scope to discuss what days are worked at interview stage. We promote hybrid working, including remote working and base working at Forston Clinic, Dorchester.
Key Responsibilities
* Undertake assessments, nurse reviews, manage a small group of patients, triage referrals as they come in and take turns with other CTLs in coordinating shifts.
* Contribute to and be responsible for implementing any service changes as guided by the National Dementia Strategy and embrace the Recovery Agenda as it is developed and applied to Organic services.
* ICSD is a virtual ward and senior nursing staff must be able to process information in a variety of ways accurately and quickly.
* Assist patients in their own environment, whether a care/residential home or private home.
* Collaborate with CMHTs, social care and other agencies to benefit patients and support carers while patients are under our care and signpost for ongoing support.
Qualifications & Requirements
* Registered Mental Health Nurse or Occupational Therapist.
* Understanding of MHA, MCA and safeguarding legislation.
* Completed Prevention and Management of Violence and Aggression course; completion is a condition of employment.
* Employment requires an Enhanced Disclosure and Barring Service (DBS) check, covered by the Trust. Applicants with DBS update service may present a valid certificate.
* Ability to drive; pool cars are available.
* Current Skilled worker sponsorship not accepted.
