Job Title: Social Worker - Hospital Team Location: Liverpool Pay Rate: £46 per hour Liquid Personnel is currently recruiting for an Adult Social Worker to join its client's Hospital Team based in Liverpool. What will your responsibilities be? Discharge Planning and Coordination Assess patients' social care needs to ensure safe and timely discharge from hospital. Arrange support services such as home care, residential placements, or equipment to facilitate recovery. Safeguarding and Advocacy Identify and respond to safeguarding concerns, ensuring vulnerable adults are protected from harm. Advocate for patients' rights, especially when capacity or consent issues arise under the Mental Capacity Act. Emotional and Practical Support Provide emotional support to patients and families coping with illness, trauma, or bereavement. Offer guidance on housing, benefits, and community resources to help patients navigate life post-discharge.
