Overview

You will be providing nursing care for palliative and end of life patients, domiciliary and ambulatory wound care, catheters, leg ulcer care and minor injuries. As a band 6 Community Nurse Specialist you will work clinically and provide day to day leadership to the team. The role involves autonomous, lone working, using specialist assessment skills to plan, implement and evaluate programmes of care for individual patients. You will work as part of a team, liaising with other professionals such as specialist hospice nurse, social workers, GP, Podiatry and Tissue Viability. We run our own well-leg/leg ulcer service, creating development opportunities for staff in leg ulcer treatment. The Band 6 role is patient-facing offering hands on clinical support and guidance. You will work under the direction of the Community Nurse Team Manager and deputise for them in their absence. The role will be working in the community and also providing clinic services 3 days a week.

Responsibilities

* Provide nursing care for palliative and end of life patients, including domiciliary and ambulatory wound care, catheters, leg ulcer care and minor injuries.
* Lead day-to-day team activities and work clinically with autonomous practice.
* Plan, implement and evaluate programmes of care using specialist assessment skills.
* Liaise with professionals such as specialist hospice nurses, social workers, GP, Podiatry and Tissue Viability.
* Develop skills in structured clinical assessment when treating minor injury patients.
* Deliver wound care for the local GP practice, Minor Injury Service, catheter care, IV line care and well leg/leg ulcer care within community and clinic settings.
* Work in the community and provide clinic services 3 days a week.
* Participate in the holiday-area context of service delivery (Lynton) and support flexible staffing as required.


Location and Working Pattern
