Locality Specialist Community Public Health Nurse - Health Visitor

We are seeking highly motivated Specialist Community Public Health Nurses to join our 0-19 Healthy Child Programme in Norfolk on a fixed term contract for 12 months.
As part of a dynamic team, you will lead the delivery of our Healthy Child Programme, implementing planned programmes of care and evaluating care delivered, liaising with other agencies and delivering face‑to‑face contacts to support children and young people aged 0‑19 and their families.
Main duties of the job
As part of the Healthy Child Programme, Specialist Community Public Health Nurses work within a defined locality or within our Single Point of Access, assessing need, delegating work within the team, implementing planned programmes of care and evaluating care delivered.
Provide a child centred, inclusive, holistic and accessible Public Health programme to children, families and communities, establishing links with Early Childhood and Family Service, Early Years Settings, Early Help, other Health providers, communities and agencies (Education, School Services, Health and Voluntary organisations).
Promote the physical and emotional well‑being of children and young people, supporting them to establish good health now and in the future. Promote early intervention, health promotion, advocacy, empowerment, inclusion and reduce health inequalities to ensure children reach their full potential.
Job responsibilities
* Work as part of a multidisciplinary 0‑19 team to identify health needs and develop strategies.
* Support the 0‑19 service offer, including migration training within 2 years to other SCPHN pathways.
* Assess, plan, implement and evaluate packages of care using knowledge and skills.
* Be competent as a non‑medical prescriber.
* Support families through home visits, digital drop‑in clinics, video contacts.
* Support children and young people and families with emotional and behavioural needs.
* Advise client group on health promotion and prevention.
* Link practice with local and national targets and deliver public health programmes based on current priorities.
* Plan evidence‑based health promotion programmes; order resources; compile teaching plans; evaluate sessions.
* Act as a health link for other agencies, working closely with partner agencies.
* Work with Locality Leadership Team to support vulnerable or at‑risk children and families; adhere to safeguarding procedures; participate in child protection case conferences.
* Contribute to development of local policies and relevant service design projects.
* Take responsibility for own clinical practice, including supervision; adhere to NMC code of conduct.
* Support delivery at the Single Point of Access in Just One Number; maintain effective relationships and continuity of services.
* Support other localities and maintain clinical skills; maintain effective working relationships.
* Support students in NHS locality teams; provide practice examples.
* Be competent and confident using digital technology and promote its use to service users.
* Promote early intervention via Just One Number and Just One Norfolk.
* Use virtual platforms for assessments, interventions, training, and networking.
* Travel independently across Norfolk; hold a full driving licence and access to transport.
Communication
* Communicate complex and sensitive information to child, parents/carers and young people.
* Communicate effectively within the multidisciplinary team and model team working.
* Develop links with ECFS, educational settings and other agencies.
* Work closely with professional teams and stakeholders.
* Act as first line response to complaints within national and corporate guidelines.
* Contribute to training of students in community experience programmes.
Patient and Public Engagement
* Ensure EEC responsibilities to engage and consult with patients and the public are met in line with statutory requirements.
Budgetary Responsibility
Demonstrate responsible use of resources with awareness of budgetary restraints; ensure health monitoring equipment is maintained and calibrated.
Human Resources
Contribute to training of students and take part in appraisal and KSF process; delegate work to team members with clarity on feedback mechanisms.
Research & Development Activity
Contribute to clinical audit and research highlighting unmet needs; maintain Health Profile of the practice population.
Clinical and Practice Governance
Observe strict confidentiality in line with Data Protection Act; share data only within Trust policies; adhere to risk management processes; follow infection control policies.
Must be registered with the NMC and maintain professional registration; undertake mandatory training; participate in clinical audits; provide patients and families with information on standards; participate in clinical supervision; participate in emergency preparedness processes.
Person Specification
Qualifications & Training
* RGN/RMN/RSCN/RN - LD or Registered Midwife
* Mentor/998 or equivalent teaching qualification
* Management and leadership qualification
* V100 Non‑medical prescribing or willing to work towards
* Evidence of completion of migratory portfolio to demonstrate dual qualification or willing to work towards

* High level of clinical skills with ability to identify, respond to and evaluate health needs

* Ability to communicate effectively with children, parents and Early Years settings and other health organisations
* Ability to use Systm One
* Experience with delivering healthcare interventions via digital platforms
* Leadership and line management skills
