Geriatric Medicine Outpatients Nurse – NHS Hospitals
Location: Mid West Wales (NHS Outpatient Clinics)
Contract: Part time Flexible Weekend Work

Key Responsibilities
* Supporting consultant-led geriatric & frailty clinics
* Completing comprehensive patient assessments, including cognitive and mobility reviews
* Managing Patients With Common Geriatric Conditions Such As Frailty & falls risk
* Dementia & cognitive impairment
* Multiple long-term conditions (multimorbidity)
* Polypharmacy concerns
* Mobility issues & deconditioning
* Continence issues
* Supporting coordination of tests, referrals & community follow-up
* Providing patient and family/carer education
* Supporting telephone/virtual follow-up clinics
* Accurate record-keeping within NHS electronic systems
* Working closely with Consultants, Frailty Teams, Physiotherapists, OTs, HCAs & Social Care teams
Requirements
* Registered Nurse (NMC PIN required)
* Minimum 1 year experience in Geriatrics, Frailty, General Medicine, or Community Nursing
* Strong understanding of elderly care pathways
* Excellent communication, compassion & patience
* Ability to manage complex cases and vulnerable patients
* Confident working both independently and as part of an MDT
* You must live in the UK and have the right to work in the UK
