To ensure the highest standards of service are provided to the residents of Buckinghamshire, Advanced Practitioners will ensure:
1. Work in accordance with the Better Lives framework
2. Promote and deliver goal-based support people to achieve greater independence
3. Promote equality as an integral part of a role and treat everyone with fairness and dignity
4. Provides a proactive personalised approach to the delivery of care and practice
5. Respond to peoples needs in a timely manner

7. Give people greater choice and control over the care that they receive
8. Take account of peoples risks and safety at all times, identifying mitigations to reduce that risk
9. Support people to achieve the outcomes that are most important to them using all available resources and taking responsibility for the public purse
10. Provide personalised information and advice, safeguarding, assessment, support planning and review to individuals/families and their carers
Mobile:
A mobile role gives you the flexibility to schedule your own diary, so you can meet with colleagues and clients in a variety of locations. Youll access digital folders and resources on your laptop and be able to use the network wherever you are based. Working from home will be common, as well as meeting your team in the office where necessary.
Main duties of the job
Naturally, youll be a great people person, familiar with working to a strength-based approach. Thinking outside the box, youll support your clients and help prevent, reduce or delay the need for care. Bringing your in-depth knowledge of the latest legislation and statutory guidance, youll work independently and with colleagues to support adults and their families. Persistence and an adaptable approach are needed in an outcome-focused way with partners such as health services, community sector, housing, care providers and other stakeholders.

About us
Despite the challenges that Adult Social Care has faced within the last 12 months, our approach in Buckinghamshire has not been to take shelter and wait for the storm to pass. Instead, we have chosen totransform and restructure Adult Social Care so we can respond to a crisis as well as build for the future. We have created more roles in Social Work and Occupational Therapy; we are adapting to work much more closely with the community andsupport people near to where they live; we have developed a newcareer progression framework to encourage staff ambition and development; finally, to establish the conditions to deliveroutstanding practice, we have reduced the numbers that managers supervise and the caseloads the team have, to allow for more contact with our residents.
Initiated in 2019, our strength-based practice model, Better Lives, has been improving outcomes linked to independence, developing social connections, and improving areas that are important to the residents we support.

Job description
Job responsibilities
To be an ambassador and leader across social work or occupational therapy both internally and external to the department. To ensure the highest standards of service are provided to the residents of Buckinghamshire.
11. Work directly with adults and their families to establish their lived experiences, wishes and feelings, using a variety of techniques, as required.
12. To promote independence, wellbeing and a community assets-based approach to social care/ occupational practice at all stages in the customer pathway
13. To ensure compliance within the Buckinghamshire Multi-agency Safeguarding Procedures and that staff providing support to ensure Safeguarding timescales are adhered to where they are not, escalate evidence of ineffective safeguarding arrangements
14. Line management responsibility for newly qualified social workers/ occupational therapists and provision of professional reflective supervision for staff. The post holder will provide leadership, motivation, regular supervision and development to those staff for whom they are responsible in line with Directorate’s Supervision policy
15. Undertake specialist assessments and lead on enquiries
