We are looking for a Nurse Practitioner to join our team.
As part of a multidisciplinary team, we aim to assess, diagnose, treat, and manage the needs of our patient demographic to improve community health. Working in our modern, purpose-built medical centre with a diverse practice population of approximately 19,000, you will have a dynamic work environment.
Our Nursing team includes 5 HCAs, 5 Practice Nurses, and 3 Nurse Practitioners, managed by a Nursing Team Manager and overseen by a Clinical Services Manager. We are seeking a Nurse Practitioner for 28.5 hours per week. See hours below.
The practice provides teaching for medical students, participates in research studies, and engages in service transformation pilots.
Main duties of the job
Working autonomously to triage, diagnose, and treat patients presenting with acute illness or injury and advising patients on their ongoing medical and nursing needs.
Job Requirements:
* Offer face-to-face assessment and telephone triage for patients with acute illness or minor injuries.
* Complete E-consults within your scope of practice.
* Conduct weekly telephone care home ward rounds.
* Assess, diagnose, and treat patients of all ages (from 8 weeks old) presenting with acute illness.
* Advise patients on general health care, arrange investigations, or refer to other primary care team members as appropriate.
* Assess and treat minor injuries.
* Work according to Practice protocols, PGDs, and relevant guidelines.
* Participate in the induction and training of GP Registrars, students, nurses, or staff.
* Provide nursing cover when needed and undertake home visits as required.
* Ensure electronic patient records are updated immediately following consultations.
Person Specification
* Strong history taking and physical assessment skills.
* Team player with a flexible approach.

Additional responsibilities
Autonomously triage, assess, diagnose, and treat patients with acute illness or injury, and advise on ongoing medical and nursing needs.
Manage exacerbations of chronic diseases, providing information on diagnosis, prevention, and management to promote patient ownership.
Participate in practice-based research projects and audits as required.
Qualifications
* V300 independent prescriber is essential.
* Experience in General Practice is desirable.
