An exciting opportunity has arisen for a motivated and enthusiastic individual who wishes to apply for this Social Work Assistant Practitioner role, working within the Integrated Team to provide care and support to patients in their own homes and other community settings across Caithness.
The successful applicant will support adults with complex needs due to conditions such as learning disabilities, substance misuse, mental health issues, physical illness/disability, acquired brain injury, dementia, or old age. The post will include completing needs led assessments and formulating care plans, financial assessments and securing funding for care packages.
Applicants should hold a degree level qualification in an appropriate discipline or be able to demonstrate appropriate qualifications and experience to meet the requirements of the post.
The base for this post is in Wick but you will be expected to travel across the whole of Caithness.

This is a permanent part-time post working 29.7 hours per week, worked in accordance with service requirements.
