Lincolnshire Community Health Service (LCHS) – Registered Nurse
Lincolnshire Community Health Service (LCHS) has a vacancy for a Registered Nurse to join the South Lincs Community Nursing Team. We have one full‑time position.
As a community staff nurse you will work autonomously and as part of a team, delivering care to patients in their own homes, care homes and clinic settings. You will assess care needs and plan, deliver, and evaluate nursing care using innovative evidence‑based practice.
Community nursing allows therapeutic relationships to be developed and embodies the person‑centred care approach, placing patients truly at the centre of care delivery.
Required skill set includes:
* Wound management
* Continence care
* Palliative care
* Medication administration
* Long‑term condition management
Close collaboration with the multidisciplinary team (Social Workers, GPs and Allied Health Professionals) ensures effective coordination of care and a positive impact on patients.
Travel to patient homes and clinic settings is essential. Due to the rurality of Lincolnshire, access to personal transport is required.
The service operates 8:00 am to 6:30 pm, seven days a week, with variable shift patterns: Early 8:00‑4:00, Mid 9:00‑5:00 and Late 10:30‑6:30.
A 12‑week induction programme, robust preceptorship and clinical skills programme provide peer support and comprehensive training.
