An exciting opportunity has arisen within Lichfield & Tamworth, working with Multi-Disciplinary team members including Social Workers, Social Care Assessors, Nurses, Therapists, Home First, and others.
You will be responsible for supporting the delivery of the personalisation and social inclusion agenda, in line with the principles of the Care Act 2014. We are looking for an enthusiastic, proactive, and innovative individual who has excellent communication and organisation skills.
This is a dynamic post that requires you to work closely with multi-disciplinary teams and agencies in addressing a range of care/support issues, enabling individuals to remain in their own homes through the promotion of independence; preventing unnecessary hospital admissions by improving health and wellbeing.
You will have a flexible approach and be a team player with the ability to forge good working relationships. You will work within principles including Recovery and Personalisation, empowering individuals to take control through personalised, strengths and asset-based assessment and intervention, and putting them at the centre of everything. You will also recognize the rights of all to access facilities within their local community.
The role involves assessing, accessing, and implementing social care services, and providing guidance, advice, and information for adults with physical health, mental health needs, and for older people and carers.
Main duties include:
1. Completing statutory assessments under the Care Act 2014 to facilitate safe and timely discharge from D2A pathways within the community and hospital setting.
2. Assessing, accessing, and implementing social care services, and providing guidance, advice, and information for adults with health needs and for older people and carers.
3. Maintaining a flexible approach and forging excellent relationships with multi-disciplinary teams and partner agencies.
4. Working within principles including Recovery and Personalisation.
5. Empowering individuals through personalised, strengths and asset-based assessments and interventions, ensuring they are at the centre of care.
6. Recognizing individuals' rights to access valued roles, relationships, activities, and facilities within their community.
