As a Nursing Associate Band 4, you will join our innovative and forward-thinking team of Nurses and Healthcare Assistants, where your growth and development truly matter. As community services evolve, we remain committed to a responsive, patient-centred approach. You'll collaborate closely with a range of services and care for a diverse group of patients, helping deliver the right care, in the right place, at the right time.
Nursing Associates play a vital role in delivering compassionate, person-centred care across all stages of life. Guided by the NMC Code and core nursing values, they bring knowledge, skill, and empathy to support individuals in a variety of settings. As part of the community nursing team, the post holder will provide high-quality care to patients on the district nursing caseload, working both independently and alongside registered practitioners. Building on their Nurse Associate Foundation Degree, they will carry out delegated duties with professionalism and kindness, always working within their competency framework and following best practice to meet the needs of the local community.
As a Nursing Associate Band 4, you will be responsible for:
1. Adhering to NMC Standards of Proficiency for Nursing Associates, maintaining professional and safe practice.
2. Managing an allocated caseload, working collaboratively within a multi-skilled team to plan and prioritise care effectively.
3. Utilising appropriate assessment tools to evaluate patient needs and risks, including safeguarding and mental health concerns.
4. Offering health promotion advice to support patient wellbeing, covering areas like diet, smoking, and alcohol use.
5. Recognising when to escalate risks and following local and national protocols to ensure high standards of care.
6. Confidently using digital systems to access, record, and share clinical information within interdisciplinary teams.
7. Balancing workload efficiently and delegating aspects of care safely when appropriate.
8. Supporting care delivery for individuals with both routine and complex needs.
