AS A REGISTERED NURSE (RN), CASE MANAGER, you'll be responsible for completion of reports such as performance improvement reports, periodic review of departmental polices and procedures. You'll also provide Case Management and Social services to specific hospital units, as assigned or per need. You'll act as a liaison between members of the Case Management team and representatives from other organizations and providers such as Outpatient Clinics, Home Health Agencies, Long Term Care facilities, Medical Equipment Providers and Area Agencies on Aging.
Qualifications
* Must have a valid RN License in the commonwealth of Pennsylvania
* Must posses evidence of effective management, financial and leadership skills
* Must have the ability to plan and direct the department's programs and to work effectively with other disciplines
* Must have knowledge of community referral resources
* Must have knowledge of management concepts and performance improvement
* Must be a team player
