Summary: The Primary Care Therapist operates within a grant to connect children with Serious Emotional Disturbance (SED) in primary care settings to behavioral health. Therapist will provide direct services such as immediate, on-site referrals, assessment, evaluation, consultation, and therapeutic services. Access to primary care clinic EHR will help guide continuity of care between primary care and behavioral health providers. The Primary Care Therapist will offer behavioral health education to primary care staff to inform care. This person will collect, maintain, and monitor program data for monthly grant reports submitted to the funder. Education Requirement: Master’s degree in Psychology, Human Services or related activity from an approved university training program. Must be eligible for any/all credentialing required by regulatory and funding sources. Experience Requirement: Experience with seriously and persistently mentally ill consumers strongly preferred, as is experienced in a publicly funded service setting. Licensure/Certification Requirement: Current license in a Behavioral Health area (Psychology, Counseling, Social Work, Martial and Family Counseling, etc.) preferred. Key Job Responsibilities: Develop relationships with primary care providers Collaborate with internal staff and external providers/partner agencies to support continuity of care and program operations. Provision of face-to-face clinical services to clients. Said services may include individual therapy, family therapy and/or group therapy. Services are to be targeted to those clients who do not need home-based case management and support services. However, case management services shall be delivered as needed as part of the counseling intervention. Said case management services may include phone calls, writing of letters, collateral education and support, referrals and other advocacy activities necessary to support effective clinical intervention and to improve the client’s mental health status. As a sole employee of the Grant funded Program, he/she will also be required to provide and follow all grant requirements including timely reports to funder. To participate in the development of treatment plan, therapy progress, and discharge planning. Gather and maintain data relating to grant project activities and program performance To make recommendations regarding the treatment of clients. To provide clinical backup for other Mental Health Specialists. To comply with policies and procedures as outlined in Alliance Healthcare Services policy and procedure manual. Other duties may be assigned by supervisor(s) to meet the needs of the consumer and Alliance Healthcare Services. Knowledge/Skills/Abilities: This person must also demonstrate an ability to work effectively in a treatment setting dealing with emotionally or socially disturbed individuals. One must demonstrate administrative and clinical ability including the ability to evaluate work and program effectiveness. The Primary Care Therapist must demonstrate an ability to use mature judgment in problem resolution, to function creatively in evolving appropriate treatment strategies for Center clients to operate effectively in a multi-disciplinary setting. Excellent writing skills are required as is the ability to conceptualize and document clinical treatment goals, activities and outcomes. Supervision Provided by this Job: None.
