Band 6 Senior Registered Nurse - Community Palliative Care

We are excited to offer a rewarding opportunity for a Senior Registered Nurse to join our dedicated Community Team. Our Hospice Community Team care for patients in any setting, wherever they are living. Main duties of the job

As a Senior Registered Nurse, you will lead by example, providing clinical expertise and ensuring the highest standards of person-centred nursing care. You will play a vital role in coordinating complex care needs, supporting patients and their families with compassion, and promoting a safe, seamless, and supportive care environment.

Key Responsibilities

* To provide skilled nursing care to patients in need of specialist palliative end of life care, and support loved ones pre and post bereavement.
* Provide proactive, safe, and seamless quality nursing care to patients, co-ordinating their care needs facilitating a supportive, caring and compassionate environment.
* To demonstrate high standards of person-centred care and leadership of clinical expertise, acting as a mentor to supervise members of the nursing team including students and volunteers
* Manage own caseload and participate in the supervision and co-ordination of the work of nursing staff on a shift by shift basis.

Job responsibilities

JOBPURPOSE

Responsible to provide skilled nursing care to patients in need of specialist palliative end of life care, and support loved ones pre and post bereavement.

Provide proactive, safe, and seamless quality nursing care to patients, co-ordinating their care needs facilitating a supportive, caring and compassionate environment.

To demonstrate high standards of person-centred care and leadership of clinical expertise, acting as a mentor to supervise members of the nursing team including students and volunteers

Direct line management of a nursing team to delegate day to day work, conduct annual performance and development reviews and absence management with return to work interviews.

KEYACCOUNTABILITIES & RESPONSIBILITIES

Clinical Practice

Be professionally accountable for the maintenance of standards and professional practice as set by Hospice Policies/Procedures, the NMC code of Professional Standards and relevant professional guidelines such as NICE guidance and Palliative Adult Network Guidelines (PANG 2016), Palliative Care Formulary(2020)

Based on principles of best practice and clinical evidence, give skilled nursing care for End of life care, chronic disease management, and symptom control, with assessment of the physical, psychological and social care needs of patients.

Puts quality and safety at the heart of practice by delivering evidence based individualised and personalised care through holistic needs assessment, care planning and evaluation of care interventions and promotion of self-care.

Act as the patients advocate to facilitate the patients own choices promoting informed patient preferences.

To identify and record new problems and other relevant information clearly escalating as appropriate.

To ensure the safe handling of body fluids and contaminated sharps in accordance to the Hospice policy.

Support patients and their carers with managing their medicines and undertaking medicines management reviews, teaching and supporting carers to give sub cutaneous medications.

Undertake tasks such as setting up and maintaining syringe drivers, intravenous therapies, providing sub cutaneous medication, continence management and wound care

Where appropriate initiate referrals or taskings, to ensure adequate care and support to patients and their loved ones.

Provide information and support to patients and their loved ones at end of life, including help with the practical, emotional and financial impacts of supporting someone who is dying.

To advise patients and their loved ones and support them in times of stressful and emotional events that can be emotionally demanding to the nurse.

Facilitate advance care planning discussions, as part of the wider multi-disciplinary team.

Ensure effective communication with the wider healthcare services by completing CHC applications and being regularly involved in planning discharges and caseload reviews.

Undertake additional administration duties such as Controlled Drug destruction and the ordering of NRS equipment

To have knowledge of and be able to effectively use local services and resources to enable the appropriate signposting to other appropriate services to promote and enhance patient care.

Leadership/Service Development

Be alert to the needs adults at risk including risk assessment and taking appropriate action when required. Escalating any safeguarding concerns.

Participate in the supervision and coordination of the work of nursing staff on a shift by shift basis.

Work in collaboration with the Multidisciplinary Team and actively participates in multi-professional meetings as well a team, locality and other relevant meetings, acting as patient advocate and representing nursing views.

Be a member of working parties and groups considering aspects of the service and professional practice as required.

Acts as a role model for excellent communication skills and nursing expertise.

Training and Education

Participate and attend all mandatory training sessions in accordance with Hospice policy

Maintain own clinical expertise and responsible for own Continuous Personal Development ensuring meets with the NMC standards of revalidation.

To participate in an annual performance and development review with their line manager so that an agreed training programme can be developed.

Promote an informative educational environment by coaching staff within their induction program and the continuing education of newly qualified staff / new Hospice staff / and student nurses.

Participate in formal or informal teaching sessions to cascade information to other registered nurses and Health Care Assistants.

To facilitate a learning environment for students and act as mentor, practice assessor and promote Best Practice

Audit and Research

To participate in service development activities, such as audits and surveys to identify areas for improvement.

Supports and contributes to the development of policy and clinical guidelines within their area.

To access and implement as necessary all St Wilfrids Hospice policies/protocols and guidelines. To undertake a link/champion role within own area of expertise such as infection control, manual handling, falls management or medicines management.

KEYWORKING RELATIONSHIPS

The main internal and external contacts the post holder will work withincludes:

Patients and their loved ones To be able to communicate effectively and sensitively when imparting information that could be highly stressful and emotional. Ensuring effective and honest communication considering the level of understanding, culture and background

Line Manager and Colleagues - Communicates clearly and regularly discussing, reporting and evaluating patient care. Working collaboratively to share information and lessons learnt

Hospice Multidisciplinary Team and Volunteers Work collaboratively across all disciplines and departments to provide timely quality specialist advice, care and information to patients and their loved ones ensuring care is effective and coordinated.

Senior Management Team and the Registered manager Work in collaboration to ensure patient quality and safety is a high priority.

External agencies such as Echo Hub, SCFT and Primary Care, Hampshire Community Trust, Continuing Health Care, St Richards Hospital and Nursing Homes- Establish and maintain good working relationship with colleagues, so as to provide a comprehensive service to patients and loved ones to enable good liaison and share best practice.

Students across all disciplines providing a learning environment by supporting and providing formal or informal teaching

Trustees and donors ensuring a positive message is communicated about clinical services


Person Specification


Experience

* Relevant post registration experience, at Band 5 or above, in cancer, palliative care or associate speciality such as care of the elderly
* Advanced communication skills to facilitate end of life care conversations and advance care planning including contributing to the ReSPECT documentation.
* Evidence of continued professional development


Qualifications

* Current RN Level 1 registered with the NMC
* Post-basic qualification in cancer/palliative care or relevant subject or working towards
