We are looking for a skilled and reliable PharmacyTechnician to join our supportive and friendly General Practice team. Our emphasis is keeping prescribing safe,accurate and efficient, and you will play an important role in the supporting theteam. You will work closely with ourGPs, clinical pharmacist, 4 pharmacy technicians, nurses and administrativestaff, where everyone works together and communication is open and respectful.

The post is for 5 hours per week which may suit someone who alreadyworks elsewhere and wishes to increase their hours.

The role would suit someone who enjoys being part of a team,has an excellent eye for detail, takes pride in high quality work and is committedto safe and consistent patient care.


Main duties of the job

Towork alongside the practice pharmacist to maximise safe, effective andefficient systems to ensure best practice prescribing.

Tohelp patients get the best from their medicines through:

effectivemedicines reconciliation after discharge and on other transfers of care,

medicationuse reviews,

identificationof patients suitable for electronic repeat dispensing,

promotionof online ordering,

reducingwaste,

providingassurance of monitoring of high risk drugs, and

handlingof prescription queries within their areas of competence.

Tosupport the training of practice medicines co-ordinators to develop effectivepharmacy administration within the practice, especially with relation to repeatprescribing systems and processes.

To encourage the development of better understanding of the principles of medicines optimisation throughout the practice team and promoting good practice in line with therapeutic developments.


About us

Culverhay Surgery is a well-established friendly GP trainingpractice located in Wotton under Edge, Berkeley and Frampton on Severn inGloucestershire.

The Culverhay ethos is very much based around continuity of careand good team working to provide excellent, responsive patient care for our18000 patients.

Our team consists of 4 partners (1 managing partner), 13 salaried GP's, a full mix of nurses, a senior clinical pharmacist, pharmacytechnicians, frailty care co-ordinators, social prescriber and practicecounsellors. Not forgetting our fantastic reception and admin team who workvery hard to minimise administrative burden for the clinical team.

Staff turnover is very low and the practice encourages feedback,to help us create a happy supportive practice environment.

Weare a GP training practice (Severn Deanery) with 2 GP trainers. In addition, weare a teaching practice (University of Bristol medical students andUWE/University of Gloucestershire nursing students)


Job responsibilities

To support thepractice pharmacist in ensuring safe and best practice prescribing.

To undertake medicinesreconciliation in accordance with practice protocols and update patientsrecords accordingly. To include new carehome patients and the synchronization of medicines for transfer of care betweencare settings.

To improve prescribingprocesses and support the development and implementation of prescribingpolicies and guidance.

To support the practicepharmacist to provide medication reviews and patient information in differentformats as required according to the patients individual needs.

To promote electronicrepeat dispensing (eRD) and online ordering.

To provide adviceto GPs, staff and patients regarding changes to prescribing to support theimprovement of prescribing safety, quality and cost effectiveness.

To deliver training and support the development ofpractice based medicines coordinators to improve co-ordination and effectivepharmacy administration within the practice.

To support thepractice reception team in streamlining prescription queries, allowing thepractice pharmacist to review the more complex requests.

To liaise with otherpharmacy stakeholders e.g. community pharmacy, and hospital pharmacy teams.

To manage shared care protocols, liaising with thepractice pharmacist for complex patients.

To ensure that all major MHRA drug alerts, whereappropriate and within level of competence, are acted upon within the practiceover an appropriate time frame and to advice on any medication changes thatoccur as a result.

To support initiatives for antimicrobial stewardship toreduce inappropriate antibiotic prescribing.

To utilise GP practice clinical systems to conductprescribing audits and to share the outcomes of clinical audits to facilitateimprovement and changes in practice.

Toproduce patient information leaflets and posters and run medicine awarenessprojects throughout the year.

To use and maintain information systems and databases relevant to theposition including incident reporting, medicinesoptimisation, prescribing data, cost savings and patient outcomes.

To support thepractice to deliver on prescribing Quality and Outcomes Framework (QOF)targets.

To be responsible for the organisation, planning and ofown workload to meet set deadlines.

To undertake learning and development to ensure therequired knowledge and skills for practice based work

To carry out other duties which are appropriate to theskills and competencies of the post holder and grade of the post as thepriorities of the service change.

To handle functionspecific information, which may be sensitive, complex or confidential andappropriately recording, transferring and/or coordinating such information inaccordance with the Data Protection Act; Caldicott Guidelines and theConfidentiality Code of Conduct.


Person Specification


Qualifications

* Professional registration with GPhC.
* BTEC/NVQ level 3 or equivalent in pharmaceutical sciences.
* Enrol in, be undertaking or be qualified from an approved training pathway. For example, the Primary Care Pharmacy Educational Pathway (PCPEP) or Medicines Optimisation in Care Homes (MOCH).
* Evidence of continued professional development (CPD).


Experience

* Experience of working as a qualified, registered pharmacy technician in primary care, community or hospital pharmacy
