Job summary
The post holder will be the focal point for and work closely with Primary Care Network member practices and clinicians to ensure that Shropshire Rural Alliance PCN delivers the contractual elements of the Network Directed Enhanced Service (DES) contract. They will also be the main point of contact for the PCN ARRS staff by supporting them with their appointment sessions and coordinating their schedules.
Main duties of the job
The role is a mixture of administration, engagement, care coordination, supporting teams and practices in delivering the Network DES. Specialising in Care Coordination, this role places a strong focus upon working closely with GPs and other Primary Care professionals across the PCN.

Job description
Job responsibilities
The following are the core responsibilities of the PCN Care Coordinator. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon other factors such as workload and staffing levels.
1. Provide support and guidance to ARRS staff in the running and operation of their appointment sessions. This will include liaising with the Paramedic team to coordinate their route planning for the day.
2. Coordinate ARRS staff annual leave requests, ensuring there is appropriate cover in place.
3. Coordinate any ARRS appointments that need to be re-arranged by liaising with the member practices and patients.
4. Support the Paramedic team with equipment issues/stock control, reporting to practices following visits, ensuring any urgent messages to or from the Paramedics are passed to the appropriate duty doctors at each practice.
5. Report and communicate activity on a regular basis to the PCN Clinical Director and PCN Managers to ensure our on-going compliance with the Network Contract DES.
6. Support in staff inductions, by coordinating IT training and following this up with appropriate check-ins when needed.
7. Develop up to date staff training aids for PCN staff to encourage the efficient utilisation of appointments.
8. Support with additional projects occuring within the PCN. This could include work around health inequalities, health promotion and overall patient care.
