The service operates from 8am to 8pm, seven days a week, offering flexibility in working hours. An additional team of Nurses and HCAs provides a robust evening and night nursing presence.
With vacancies in Carlton, the role offers a great location with mixed rural and urban areas.
Responsibilities
* Provide skilled nursing care and work flexibly to support the team in delivering high-quality, integrated care.
* Demonstrate excellent communication and interpersonal skills; be adaptable, reliable, resourceful and approachable.
* Deliver integrated care following holistic assessments to enable patients to be cared for in their own homes and clinic settings.
* Facilitate the prevention of unnecessary hospital admissions/re-admissions by reducing ill health and disability and maximising independence.
* Engage in robust induction and competency-based development.
* Assess, plan, implement, promote and evaluate safe, creative and effective treatment plans with patients, carers and other professionals.
* Deliver evidence-based interventions and be responsible for safe monitoring, management and administration of medication.
* Build effective relationships with patients, carers, families and colleagues across the MDT; develop planning and organisational skills.
* Develop sound clinical decision making and autonomous leadership capabilities; delegate and support junior staff as appropriate.
* Develop knowledge in areas including palliative care, leg ulcer and wound care, bladder and bowel dysfunctions, and supportive care for carers and families.
Requirements & Qualifications
* Registered adult nurse with current UK registration and a willingness to work in community settings.
* Ability to provide high-quality care in diverse settings, including patient homes and clinics.
* Full UK driving licence and vehicle for business use (reasonable adjustments available per Equality Act 2010).

* A robust induction and competency-based development package will be provided.
* Paying for DBS check: cost (£26.40 standard, £54.40 enhanced) deducted from salary over first 2 months; DBS Update Service encouraged (£16/year).
