Overview
We have an exciting opportunity for a permanent part-time (3 days a week / 22.5 hours per week) Social Work Assistant in the Disabled Young People’s Team covering North Kent. The team covers Dartford, Gravesham, Swanley, Swale and surrounding villages. We have Gravesham Civic Centre and Orchards MASH, Swale as our office bases. We promote hybrid working but do encourage working in an office at least twice per week so the team can get together and share ideas and knowledge whilst supporting each other. There is an expectation that you are able to work flexibly across the whole area covered by the team.
We are able to provide an extensive training and development programme and you will be provided with support and opportunities to further develop your own knowledge and skills with regular supervision.
Role and Responsibilities
* Hold your own caseload of young people aged 18 - 25 with a range of care and support needs, supporting them as they transition into adulthood and helping develop their skills and independence.
* Complete direct work with young people in a variety of settings to ensure the young person’s voice is heard and promote their safety in day-to-day life, helping them live as fully and independently as possible.
* Work alongside and in partnership with other professionals to develop plans to meet the identified care and support needs.
* Ensure all record keeping is accurate and kept up to date using the appropriate IT systems.
* Demonstrate motivation and excellent interpersonal and communication skills to deliver a high-quality service to disabled young people, their families and carers; work collaboratively as part of the team.
Qualifications and Requirements
* A qualification of Level 2 Health and/or Social Care or equivalent is required for this post.
* Experience in either Adults or Children's Services is essential.
* This post is subject to an Enhanced Disclosure Application to the Disclosure and Barring Service (DBS).
* This is a customer-facing position. The Council has a statutory duty under Part 7 of the Immigration Act (2016) to ensure that post holders have a command of spoken English/Welsh sufficient for the job requirements. The standards are set out in the Job Description/Person Specification.
