Overview
Responsibilities
* Conduct Care Act assessments to tailor support for adults and carers
* Design strength-based care and support plans
* Engage in capacity reviews and Best Interests meetings
* Handle safeguarding responsibilities with a focus on protection and intervention
* Collaborate closely with a multidisciplinary team including Occupational Therapists, Nurses, and Mental Health Practitioners
* Execute all associated administrative tasks, attend case conferences and represent the Council in court as required
* Provide comprehensive assessments, leveraging resources from social care, health, and other sectors
Qualifications
The successful candidate will have experience and expertise in care act assessment, strength-based support planning, BIA and capacity reviews, safeguarding, and experience working in a neighbourhood team within Bolton. Candidates should possess either 2 years in a specific neighbourhood team or 3 years in a related area and must be local to Bolton for office attendance when necessary.
