BloomfieldMedical Centre is a 7 Partner GP Practice Primary Care provider based over twosites in Blackpool, Bloomfield Medical Centre on Bloomfield Road and GrangePark Health Centre on Dinmore Avenue.Our services are diverse, forward-thinking enabling us to deliver careto meet local priorities in relation to deprivation, chronic disease, improvedaccess, migration from secondary care and more efficient commissioning.

Weprovide high quality general medical care to a registered list of over 16,000patients over the two sites, through our dedicated team of clinical,administrative and management professionals including 7 GP Partners, Salarieddoctors, Clinical Pharmacists, Advanced Clinical Practitioners, NursePractitioners, Practice Nurses and Healthcare Assistants.


Main duties of the job

Provide care to patients, including those with undifferentiated and undiagnosed health problems

Provide continuing care to patients across the whole practice population

Provide a range of appointment types including e-consultations, video, telephone and face-to-face

Experience in managing patients with complex needs and minor illnesses

Provide excellent clinical examination skills and communication skills

Applicants must have completed, in full, an advanced clinical practice qualification to post graduate or masters level including independent prescribing and have sufficient experience to work autonomously and independently within a wider team.

Act as an independent prescriber (preferred, though will support training and qualification if needed)

Able to demonstrate safe, clinical decision-making and expert care for patients within the general practice

Deliver a high standard of patient care using knowledge base with the ability to demonstrate safe, clinical decision-making and expert care for patients

Work collaboratively with the multi-disciplinary team to meet the needs of patients, supporting the delivery of policy and procedures

Job responsibilities

* Work as an autonomous clinical practitioner accountable for own professional judgement and actions
* Consult, assess, diagnose, plan, implement and evaluate treatment/interventions and agree goals for patients presenting with an undifferentiated diagnosis through a co-production approach
* Provide support with the e-consultation service to patients submitting e-consultations (in addition to general appointment delivery types)
* Proactively identify patients at risk of developing a long-term condition (as appropriate) and facilitate the creation of their own goal setting through the development of a self-management plan
* Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug-based treatment methods into a patient management plan
* Undertake face to face assessments and develop treatment plans for patients not requiring a GP consultation
* Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within the scope of practice
* Request and interpret tests appropriate to patients needs
* Where required, undertake home visits as assessed by GP and acute visits for certain medical emergencies and arrange hospital admission for appropriate cases
* Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
* Undertake telephone triage work and telephone advice calls
* Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care
* Support and manage health needs of women presenting for family planning, cervical cytology or sexual health consultation
* Assess, identify and refer patients presenting with mental health needs in accordance with the national and local guidelines for Mental Health services
* Implement and participate in vaccination and immunisation programmes for both adults and children
* Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment
* Communicate with and support patients who are receiving difficult or complex information or results in an professionally appropriate and supportive manner
* Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating
* Organise, coordinate and lead the provision of the clinical team for the service
* Promote the service and maintain the positive reputation within the local health economy
* Lead in the administrative and professional responsibilities of the Practice team
* Ensure accurate and legible notes of all consultations, treatments and attendances are recorded.
* Ensure the clinical computer system is kept up to date.Ensure data collection requirements are maintained
* Lead and contribute in staff/practice meetings
* Support and develop close working relationships with reception, office and other staff.
* Build and develop good working relationships with other service providers,


Person Specification


Qualifications

* Clinical Advanced Practice to postgraduate level - postgraduate certificate or diploma, or full masters
* Nursing Degree Independent prescribing qualification
* Chronic disease management qualifications


Experience

* Advanced clinical practice skills
* Experience working independently and autonomously without immediate GP support (required for new models of care within a multidisciplinary team setting)
* Experience prescribing for paediatric and adult patients presenting with undifferentiated and undiagnosed problems
* Experience in General Practice / Primary Care
* Experience in training, mentoring and supervision
