Job Overview
Manage a caseload within a defined population, with the overarching aims of increasing family resilience, improving population health outcomes, reducing health inequalities, and safeguarding the wellbeing of children.
* Provide a Health Visiting service for a defined caseload and cover for other health visitors during leave.
* Utilise the All Wales Health Visiting Acuity tool to ensure safe staffing levels and support colleagues during fluctuations.
* Be responsible for personal performance, develop personal development plans, and adhere to NMC revalidation requirements.
* Promote opportunities for integrated working and contribute to local partnership plans.
* Develop and maintain health profiles of the population to identify health needs and influence service planning.
* Share responsibility for chairing Health Visiting Forums and team meetings.
* Contribute to the development of local policies and guidelines.
Main duties of the job
This is an exciting opportunity for an experienced and enthusiastic Health Visitor to join the Blaenau Gwent Early Years Team. The roles will provide the Early Years/Flying Start Core Programme to all families in the area and work within a well-established multiagency team.
The ability to speak Welsh is desirable; Welsh and/or English speakers are equally welcome to apply.
