We are looking for enthusiastic, skilled and motivated Community Healthcare Support Workers to join our Quickstart service across Wandsworth and Merton boroughs to deliver high quality, timely care support with an enablement focus to patients in the community. We have 1 positions that are permanent.
Quickstart service provides rapid response social care services for patients who are at risk of social care breakdown. Without a social care package in place these patients would otherwise be admitted to hospital due to an overriding social care need.
Main duties of the job
Quickstart works closely with Maximising Independence Team, a dynamic, multi-disciplinary team of physiotherapist, occupational therapist, speech and language therapist and rehab assistants, allowing for joint working with rehabilitation and learning opportunities. The post will provide an opportunity for the successful candidate to develop their clinical skills and knowledge base.

Person Specification
Qualifications
* NVQ 3 in Health or Social Care or equivalent academic qualification
* Completed first level education, GCSE or equivalent, grades 9 - 4, including Maths and English
Experience
* Experience of working in a health or social care setting
* Competent in the use of Microsoft Office (e.g. Word, PowerPoint, Excel, and Outlook ) with experience of inputting to databases, spreadsheets and/or other IT systems and business applications used in the office environment
* Experience of working as part of a team
* Experience of working in a customer service role
* Experience with the NHS patient information database (Rio, CIS)AF/IV
Key Attributes
* Flexible about work location - the post holder will be required to travel to other CLCH sites in London
* Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)
Skills
* Knowledge of NHS
* Understanding of the need for confidentiality and management of sensitive records
* Knowledge of the effects of the ageing process and needs of vulnerable adults in relation to their health and well-being
* Able to deliver and monitor prescribed care plans
* Excellent oral and written communication and presentation skills & ability to build effective relationships
* Previous experience of working on an electronic record system
* Able to understand and promote enablement strategies
