We are looking for a self‑motivated and passionate Qualified Practitioner registered as either a Mental health nurse with the NMC, an Occupational Therapist with the HPCP or a Qualified Social Worker registered with Social Work England to join our early intervention in psychosis service in Mid Essex.
Responsibilities
* Fully conversant and uphold professional code of conduct.
* Ensure all care is delivered in accordance with Trust’s care programme approach guidelines.
* Participate in initial assessment of clients to determine appropriateness for FEP Team and liaise with referring agencies.
* Undertake risk assessments with clients and plan effectively to manage them in care plans with MDT.
* Ensure interventions are undertaken within established time scales and according to best evidence‑based practice and operational policy.
* Work flexibly and participate in the demands of the FEP Service; pattern subject to change to meet client needs and guidelines of the National Service Framework.
* Provide clinical supervision and professional leadership to junior staff and promote evidence‑based practice.
* Attend training as provided and keep oneself up to date with mandatory training.
* Keep updated in all legislative government initiatives, particularly those relevant to Early Intervention in Psychosis.
* Participate in collection of data including entry and exit pathways to care and duration of untreated psychosis, as well as using outcome measures – Dialog & QPR to ensure successful high‑quality outcomes.
* Maintain effective individualised care through assessment of service user and carers problems; plan, implement and evaluate interventions and produce clear, accurate and concise documentation of individualised care.
* Undertake initial health and social needs assessments and participate in multi‑disciplinary team meetings to review appropriate selection and allocation of clients to CPA and conduct regular reviews.
* Plan creatively to meet client needs including provision of evidence‑based interventions.
* Prioritise and manage workload to ensure effective management of a defined group of clients with complex needs.
* Work with colleagues to improve the quality of services provided to clients.
* Participate in annual audits relevant to the clinical pathway.
* Participate in first episode psychosis duty rota.
