Role Description
This is a full-time role for a Registered Care Manager located on-site in Horsham. The Registered Care Manager will oversee all aspects of client care, including case management, and provide comprehensive support to clients and their families. Daily tasks include coordinating care plans, conducting home visits, supervising and training Care Pros, and collaborating with healthcare professionals to ensure quality elder care services.

* Experience in Training and Elder Care
* Knowledge of Medicine and healthcare practices
* Excellent communication and organizational skills
* Ability to work independently and collaboratively
* Relevant certifications in care management are required
* Registration with CQC
