Join our team of GPs and ANPs inthe New Models of Care programme for the Northamptonshire Rural East &South (NRES) Neighbourhood.
Work within our practices acrossthe neighbourhood to deliver high-quality, same-day care to our patients.
We're looking for GPs to joinour Neighbourhood Access Service, part of the innovative New Models Programme.You'll work autonomously and as part of a multidisciplinary team within primarycare to provide expert assessment, diagnosis, treatment, and evaluation forpatients presenting with undifferentiated primary care presentations.
This is your chance to shapeintegrated neighbourhood services and make a real difference to patientoutcomes across our 88,938 registered patients.
We are pleased to offer flexible baselocations across our seven member practices and three hub sites. We will workwith successful candidates to identify a primary location that best suits yourpreferences and our service needs.
Main duties of the job
As a GP in the New Models ofCare programme, you will provide autonomous care for our patients presentingwith undifferentiated presentations mainly in same day settings. You willdemonstrate safe clinical decision making and provide expert care by undertakinginitial history taking, clinical assessment, diagnosis, treatment andevaluation of care, including appropriate follow-ups.
Working with other GPs andnursing team in the neighbourhood Practices and Hub you will continually reviewand evaluate services and work with the management team in implementation ofchange.

About us
The Northamptonshire Rural East& South (NRES) Neighbourhood, consisting of 7 GP Practices serving 88,938patients, is developing an innovative new model of care as part of the NHS NewModels Programme.
You will be working with a teamof GPs and specialist ANPs to deliver same day care to the Rural East &South population across our three hub sites.

Job responsibilities
Clinical
Provide clinical care to our patients. This will includereducing the demand on the core neighbourhood GP practices by the assessmentand management of patients by means of telephone triage and clinicappointments.
Use advanced skills and expert knowledge to assess thephysical and psycho social needs of patients with undifferentiated primary careconditions including minor illness and instigating therapeutic treatments basedon best available evidence to improve health outcomes.
Use advanced skills and expert knowledge to makecomprehensive and focussed assessments, independently and proactively managingcare of the patients.
Promote patient independence and self-care whereappropriate
Support and work cohesively in a team with other GPs/ANPsand ACPs working in the new model of care settings, including providingoversight and mentorship for more junior colleagues.
Review pathology results to ensure appropriate treatmentand care
Support and lead the practices for the delivery of theQuality and Outcomes Framework and NHS England, ICB, Federation Contracts
Regulatory
Respond to complaints and clinical incidents, significantevents as appropriate to ensure robust systems are in place for improvingpractice through joint learning.
Maintain accurate and timely clinical records in linewith professional and legal standards.
Undertake at least one clinical audit a year. Evaluatedata and present findings to the Practice Clinical team. Implement changeswhere appropriate.
Adhere to the employing practice Policies and Proceduresincluding Infection Control, Health and Safety, and safeguarding. Ensure allmandatory training is completed in a timely manner.

Personal and Professional Development
Maintain own personal/professional development plan andportfolio thereby meeting the requirements for revalidation through appraisalof own performance.
Adhere to the annual appraisal process and revalidationrequirements.
Work at all times within the GMC Code of ProfessionalConduct.
Provide Mentorship and training of new recruits orstudents as and when required.
Participate in the Practice Performance Review process.

Qualifications
* On the performers list register for Northamptonshire
* Extensive clinical experience in primary care
* Evidence of continuing professional development
* Post registration qualification in relevant areas
* Teaching qualification
* Recent attendance on a leadership skills development programme
Experience
* In depth knowledge base in the management of patients with chronic illness and minor illness
* Ability to maintain and monitor high standards of evidence based care
* Ability to direct and co-ordinate care working autonomously and collaboratively
* Understanding of clinical governance issues
* Evidence of improving the patient's experience
* Evidence of leading a service project at practice level
* Undertaking research or involvement in clinical research programmes
* Experience in writing clinical policies and protocols
* Excellent communication (verbal, written, and IT)
* Evidence of self motivation, the ability to work independently and effective decision making
