Job Summary
We are recruiting for a Patient Flow Co-ordinator who shares our vision to provide consistently outstanding care and exemplary service to our patients.
Key Responsibilities
* Ensure flow is created in the Trust to allow patients to be placed in the right bed at the right time, supporting patient flow and facilitating timely safe discharges.
* Support wards to unblock delays and issues that may impact timely discharges.
* Problem-solve issues that are causing discharge delays and support the implementation of care when patients are discharged home.
* Work closely with Pharmacy, Site Office and Patient Transport to coordinate discharges from the acute trust and elevate delays to senior staff.
* Manage the discharge today list and keep the Nerve Centre system updated.
* Participate in transferring patients to the Discharge Lounge early and throughout the day.
* Manage and support the home today list on Nerve Centre, ensuring patients are discharged as planned.
* Communicate and collaborate with support services, transport, pharmacy, discharge lounge, Transfer of Care Team, Site Office and ward staff as required.
* Ensure next day discharges are shared with all support services on the Nerve Centre application in real time.
* Any other tasks that fall within the role to assist with the continued discharge of patients and support the Transfer of Care Team.
Rostered weekend working is required.
