Key Responsibilities
* Perform initial and ongoing patient assessments, including OASIS evaluation and documentation.
* Develop and implement tailored treatment plans in consultation with physicians and clinicians, adjusting as needed.
* Collaborate with the interdisciplinary care team to optimize patient outcomes.
* Provide education and support to patients and families/caregivers on therapy programs.
* Maintain accurate clinical records and report any changes in the patient’s condition to the physician.
* Supervise Physical Therapy Assistants in accordance with agency policies and state regulations.
* Participate in in-services, case conferences, and meetings to promote coordinated care with healthcare team members.
* Identify patient and family/caregiver needs for additional services and make appropriate referrals.
* Confirm daily visit schedules to ensure seamless coordination with other staff.
* Communicate patient progress and changes with physicians and care teams.
* Document all visits and progress notes on the same day of service.
Qualifications
* Must have a current PT license (state-specific).
* Graduate of an accredited physical therapy program.
* Valid BLS certification.
* Minimum one year of PT experience (home health experience preferred).
* Excellent communication, organizational, and time management skills.
* Must have a valid driver’s license and access to a reliable and insured vehicle.
* Ability to visit homes that may not be wheelchair accessible.
* Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.
* Engage in physical activities such as walking, standing, stooping, and occasional lifting or turning of patients.
* Proficiency with electronic medical records is a plus.
