An exciting opportunity has arisen for an experienced Social Worker to join the Hospital Discharge Team in Windsor and Maidenhead.
This ongoing, full-time, hybrid locum role offers a competitive hourly rate of £34.00 and the chance to work in a fast-paced, supportive Adult Social Care service where you will play a vital role in ensuring the safe, timely and effective discharge of adults from hospital into the community.

What You Will Do
Deliver statutory services to adults with eligible care and support needs, ensuring assessments are person-centred, strengths-based, and outcome-focused.
Facilitate safe, timely discharges from hospital into home or community settings, working closely with health colleagues, families, and providers.
Manage a complex caseload and ensure all interventions, decisions, and plans are recorded accurately and on time.
Undertake Care Act assessments, reviews, and support planning.
Complete Mental Capacity Act assessments and Best Interests decisions where required.
Contribute to safeguarding enquiries and multi-agency safeguarding processes.
Coordinate and arrange care packages, placements, and services to meet eligible needs and prevent readmission.
Work closely with community resources and partner agencies to promote independence and recovery.
Support and oversee team members where required, helping to maintain high standards of professional practice.
Attend and contribute to multi-disciplinary meetings, ward rounds, and discharge planning forums.
Maintain high-quality, timely case recording on the council’s case management system.

Mental Capacity Act 2005
Relevant statutory guidance and local policies
Participate in supervision, team meetings, training, and continuous professional development.
Requirements
Degree or equivalent qualification in Social Work.
Registered with Social Work England.
Experience within Adult Social Care and/or Hospital Discharge services.
Strong working knowledge of:
Care Act 2014
Mental Capacity Act 2005
Safeguarding Adults procedures
Ability to work confidently in fast-paced, multi-disciplinary environments.
