Jurassic Coast Primary Care Network is looking for an organised, proactive and approachable Digital Care Coordinator to join our PCN Administration Team.
This is a varied role supporting the smooth running, promotion and development of PCN services across our member practices. The postholder will play an important part in day-to-day administration, service coordination, patient communication, digital communications, social media updates and promotional materials, while also supporting searches, reports, datasets and data quality across PCN services.
A key part of the role will involve using systems such as SystmOne, Microsoft Office and local reporting tools to help identify patients who may benefit from PCN services, monitor service activity, review uptake and support more proactive, preventative care.
We are looking for someone who enjoys being organised, working with people, using data practically, and supporting a busy team. The successful candidate will be confident communicating with patients and colleagues, comfortable using everyday digital systems, and able to help keep information accurate, accessible and up to date.
You will be joining a supportive PCN team working across Ammonite Health Partnership, Barton House Medical Practice and Lyme Bay Medical Practice., The Digital Care Coordinator will provide administrative, communication, data and coordination support across the PCN.
The role will include supporting day-to-day administrative processes, patient contact, inbox and correspondence tasks, waiting lists, clinic preparation, follow‑up actions and general service coordination. A key part of the role will involve using SystmOne, Microsoft Office, population health tools and local reporting systems to run searches, produce reports, maintain datasets, support data quality and identify patients who may benefit from PCN services or targeted support.
The postholder will help maintain accurate records, support consistent ways of working and assist with the smooth running of PCN services across member practices. They will also support the review of service activity, patient engagement and service uptake, helping the PCN use information in a practical way to support planning, improvement and proactive care.
The role will also include supporting PCN communications, including website updates, social media content, patient information, promotional materials and campaigns to increase awareness and uptake of PCN services.
They will work closely with administrators, clinicians, care coordinators and managers, helping to keep services organised, information accurate, communications up to date and patients connected with the right support.
Jurassic Coast Primary Care Network (PCN) is a group of GP practices based in rural West Dorset, working together to provide high-quality, personalised healthcare to the local community.
