Anopportunity has arisen for a dedicated and motivated pharmacist to join theHaematology Pharmacy Team at Leeds Teaching Hospitals.

Thisrole offers the candidate the chance to build significant clinical knowledgein the management of patients with haematological conditions (both malignantand non-malignant) alongside developing a range of advanced pharmacy skillsincluding clinical trials, medicines governance and finance.

Withinour team the Specialist Clinical Pharmacists are enrolled into our highlyrecommended Systemic Anti-Cancer Verification Training Programme supported bythe national British Oncology Pharmacy Association Passport. Prior experiencein these fields is not required and the support you receive from yourmentor's will be adapted based on your previous experience and currentskills.

Thesuccessful candidate will have the opportunity to work alongside leadingclinical specialists on our inpatient wards and day case units gaining skillsand knowledge valuable for future progression to advanced roles.


Main duties of the job

* Provide a clinical pharmacy service to inpatient haematology and oncologywards
* Undertake medicines reconciliation, targeted medicines review andfacilitation of discharge for our patients
* Have oversight of medicines management for both in and outpatient areas
* Undertake ongoing training to develop your competency in the validation ofSystemic Anti-Cancer Therapy (SACT) ensuring safe, effective and efficientuse and supply of haematology related SACT to the patients of Leeds andbeyond.
* Work alongside our pharmacy aseptics unit producing SACT and ParenteralNutrition to the patients of LTHT
* Support Advanced Clinical Pharmacists in other clinical work includingmedicines governance, finance and clinical trial implementation and oversight
* Take on projects and ''link roles'' with other parts of the pharmacy team suchas medicines safety or antimicrobial stewardship
* Supervise and support Foundation Pharmacists and Trainee Pharmacistscurrently on rotation within the Cancer Team

Thesepharmacists will support you in your development as a cancer pharmacist anddue to our wide range of work our team provides lots of avenues for personaland professional development.

Wehave a strong ethos of workforce development. Junior pharmacists are assigneda mentor to support them in their SACT verification training. Our ConsultantPharmacist conducts trainee driven development sessions and finally wecontinue to support our pharmacists to undertake the independent prescribingqualification with multiple pharmacist led clinics now in operation.

Job responsibilities

* Provide specialist clinicalpharmacy service to patients and the multidisciplinary team within eachrotation, as part of a team of pharmacists working in that speciality beingprofessionally accountable for their actions.
* Provide drug-related advice andinformation to patients, consultants and other health-care professionalswithin a Clinical Management Team.
* Monitor the clinical use of drugsin patients. Advising medical and nursing staff how to optimize drug therapy.Reporting prescribing and administration near misses, and risk managementissues
* Provide education to healthcareprofessionals within the speciality
* Participate in the training ofjunior pharmacy staff within the speciality
* Support the Lead Pharmacist for thearea with the production and implementation of drug related policies andprocedures, with the production of financial reports and with horizonscanning activities.
* Participate in operational servicecommitments to dispensaries, Medicines Information and aseptic services
* The post holder will be required towork evenings, weekends and Bank Holidays on a rotational basis andparticipate in the on-call from home pharmacists rota.


Person Specification


Qualifications

* Pharmacist Registrant of GPhC
* Postgraduate Diploma in Clinical Pharmacy (or equivalent)
* Non-Medical Prescriber

Experience

* Completion of training in core elements of clinical pharmacy practice as part of a clinical diploma
